Vacation Leave Information

A Vacation Leave Form must be completed by parents/guardians two weeks prior to the scheduled date of departure. A separate form is required for each vacation; please indicate for which vacation or break the vacation leave form applies (see table).

Forms can be returned any of the following ways:
(1) E-mail; send via e-mail now by filling out the form and clicking the Submit Now button to send. Please follow up with a phone call to Keith Krusz or Donna Soule 804-443-3357 extn: 3026 to confirm and authenticate your email;
(2) Fax; print the form and fax to (804) 443-3069 Attn: Keith E. Krusz, Dean for School Life; or
(3) Postal mail; print the form and mail to: Keith E. Krusz, Dean for School Life, St. Margaret's School, PO Box 158, Tappahannock, VA 22560-0158.
